Are you saying that ini.xml (http://php.net/ini) is not in the PhD
generated CHM?
I mean the name of every single directive should be in the /index/ (2nd
tab, after the big toc).

Done. Now, every single directive are in the CHM index (all the
varlistentries which have a xml:id actually).
