There have been several reports of this lately. The best I can work out is that something has changed with the $_SERVER variables somewhere in PHP5, which only shows up on some installs.
As yet I haven't been able to replicate it. It could be something to do with using PHP as a cgi, but that is as far as I have got with working out the cause of it.
