It sounds most likely like a hormone deal, although some of those symptoms also are found in problems such as Fibromyalgia and Chronic Fatigue which can Accompany hormone problems- some of the symptoms he is having might be able to be corrected through proper treatment such as either antidepressents or natural antidepressents- And I don't mean just for the depression either. Seratonin is crucial to health, and from the sounds of it your friend is suffering low levels. He should find that being on seratonin modulators will help things like his memory, his sleep patterns (Which is crucial for healing), his concentration ability etc.  But definately discuss this with his Doc first. There's a couple different types of antidepressents that work a bit differently
 
I however don't liek the sound of the sleep apnea and low testosterone- those two can be indicators of Acromegally (Don't tell him that right now though until tests come back, becasue it may be somethign else, and you don't want ot worry him unessesarily). The MRI is probably the best line right now- definately talk to the endo about it-
 
I haven't heard that going off androgel, then resuming it has problems- I wouldn't think it would cause depression, low testosterone is more likely to cause depression- But maybe do a search o nthe net for "Androgel and depression' or something and see what other peopel have to say & what clinical trials say-
