When I start the pkg manager is takes a moment to refresh the
database. but then it does not show any installed pkgs with the
check mark. And if I check a pkg to install or whatever, the
`Install/Update' button stays greyed out, as does the `Remove'

So it offers neither to install or remove anything.

I've never heard of this particular issue before. Although, I'd note
that your publisher shouldn't be named release111b; it should be
'opensolaris.org'.
I saw the same problem yesterday on b121 when it was scanning the
changes for
b122. The orange slider box on the bottom went away and the list of
categories
on the left were updated indicating updates were available,
but the Update All and Install/Update icons were grayed out. The cursor
was the
normal pointer and there was no visible indication in the window there
was any
activity still in progress. It remained in this state for about ten
minutes and then
the Update All icon was activated.
During the ten minutes, the only indication something was still
happening was
the lights on my cable modem flashed. Maybe Package Manager needs some
visual clue that it is still thinking.
FWIW, my publisher says "opensolaris.org" and is pointed to the /dev
repository.
