I have respun the webrev, http://cr.opensolaris.org/~padraig/ips-11211-v2/.
The problem was that the property 'always-show-image' property was set
on the ImageMenuItems and this overrode the setting you were changing.

As part of adding icons to the requested menu items, the icons for
install_update, reload, remove, update_all were move from the glade
file directory to the directory for the default theme.
The icons arrow.png, new_star24x.png were removed as they are no
longer used.
The icons packagemanager.png used as the application icon is now
storeed in the mercurial repository instead of being copied from
another icon.
