Bugs:
14307 writing out the main dictionary is slow
14373 indexing tracker doesn't update
This change provides about a 20% improvement in the time that building
an index from scratch takes by turning a tree of recursive function
calls into many layers of for loops.
To test this, I've diffed the files this code writes on both sparc and
x86 and verified that the indexes. produced are identical.
