The webrev below contains fixes for the following issues:
15998653 variant and mediator tagged actions with same path ignored
in one pkg

In short, if a single package contains links/hardlinks that are tagged
with both a variant and a mediator, and more than one of them shares the
same path, some of them will be silently ignored by some package
operations (such as change-variant, change-facet, update; but not
install, uninstall, set-mediator).
