It's the responsibility of the person who changes the code to update the documentation. Like in a shared kitchen: "your mom doesn't work here." Or a magical elf from Ear Mountain. If the community doesn't learn to maintain documentation properly then it either must be - Enforced by release management process - Enforced through automated tests (like plone.api does).
