You should see 6 required fields in the Advanced form.
If you're using the Simple form, they might not be there. I've just
disabled the Simple form from now on. There really is no reason to use it at
this point.
Try again, and see if it works for you. I just posted two test bugs
in a separate project, with attachments, and they both worked fine.
