Open a terminal in Ubuntu and type
lspci
This will tell you your modem chipset along with other stuff.
I had similar modem....it is a conexant chipset modem.
You get a free but speed limited driver from
www.linuxant.com called hsfmodem.
Not possible to install on knoppix live.
