I'm pretty certain that Google Voice does not use VOIP for the last
leg.  I've been using it as a personal PBX-like system, just for local
calls (I've never needed to use the google voice  paid long distance).
 So far, there is every indication that it is just routing calls to
the gvoice number over my standard phone line(s).  The call initiation
can be done either over a data connection or via phone (using tones?
There is a google voice number, or numbers, that you can call and then
issue a call from without needing a data connection.  I don't know how
the google voice app behaves.)

Anyhow, after ~2 years of google voice, I'm very happy with the
service.  Voicemail transcription is awesome, the scammer call
blocking is great, and I don't have to worry about carrying my number
over when I change carriers, or if I need to receive calls on a land
line for some reason.  You can also send text messages directly from a
Chrome plugin (so you have a real keyboard!), and the recipient is
none the wiser :)
