If you can get to the bootloader (GRUB, probably) menu and edit the
kernel commandline, the traditional (maybe obsolete) technique was to
add something like init=/bin/bash, which would give you a root shell.
You might need to mount or remount filesystems and the like to get
write access to the /etc/passwd, /etc/shadow.  Apologies for
vagueness, it isn't an every day thing, so details are rusty.  But
that was the general approach.  The Live CD/DVD (or USB) should work
about the same.
