Instead of focusing on smtpd_*_restrictions, you may want to ensure that
your relaying is correct. I personally use smtpd_*_restrictions for stuff
like RBLs, but otherwise rely on mynetworks for relaying permissions. This
takes care of preventing us from becoming an open relay.
