The SQLPlayList menu is by default a sub menu below the Plugins menu, but this can be moved to a top menu in the squeezebox settings web interface.
The playlist will appear automatically the next time you select the SQLPlayList menu so no rescan is required. It will look for *.sql files in the directory which is configured as playlist directory in the SQL Playlist settings in the web interface.
