Well, I never knew that before! I never actually invoke the mode you described but I just ran a little test of 3 songs on a playlist with shuffle turned on. After playing the first track and going into the second I added a 4th track and lo and behold the list reshuffled and the already played first track came up as the 3rd (i.e. next) track.
It would appear that unless shuffle is turned off this is bound to happen and I agree that it would be quite annoying if you were adding new tracks and kept getting repeats of those you had already heard. No idea how to fix it other than not using shuffle in which case tracks will get played in the order they are added - which may or may not be acceptable.
