Just did another clean sweep and no change. Still no images in my image
folder.

But you didn't enable debug logging for scan.scanner... no new information in your log file. Do NOT restart LMS after setting the logging options. Set the scan.scanner=DEBUG, start scan.

However, I'm still thinking the write access may be the issue. Is there
a way to test LMS ability to write to that folder? If LMS is being run
when I login, does it get all the file permissions I get outside of LMS?
If so then I guess that isn't the issue.

I'll add a log statement in the next release which would log an error if write access was not given. Right now there unfortunately isn't an easy way to see this, other than running in full log mode.

What is interesting is when I browse artist images after the clean sweep
and rescan, most of my "A" artists show images very quickly, as if its
loading from hard drive, but a lot of the images are mixed up and
reflect a different artist in my library.

That's some caching issue somewhere. Likely even in the browser, as you say you only see it on one device, but not the other.
