If SlimServer freezes after playing tracks, it's probably the case that it can't communicate with iTunes. You can verify this by enabling the --d_plugins debug flag (Server Settings->Debugging) and checking the log after the freeze occurs.
Are you running SlimServer with the 'start automatically on login' option? Reinstalling iTunes has also fixed the problem for a few people so far.
