I think your problem is now mplayer not handling your audio output and
there is no problem processing RealAudio.

>From your descriptions, this is what happens now:
1. Mplayer playing can process a RealAudio stream and save to a WAV
file.  The WAV file can be played by another device so it is OK 
2. When mplayer is used to play a valid WAV file - but output is heard.


Can mplayer play any audio file on your system ?

Try the following  - first line is audio output, 2nd line is to a file
(the -v -v -v gives more debug output).
mplayer -v -v -v -af volume=0,resample=44100:0:1,channels=2 
REALAUDIOURL
mplayer -v -v -v -af volume=0,resample=44100:0:1,channels=2 -ao
pcm:waveheader:file=output.wav  REALAUDIOURL
