No changes to the table structures, but it creates a new database index
the first time the new Custom Scan version is started, this might take
some time. Could it be that the first time you started it you thought
it hanged and killed it and it didn't really kill MySQL so it is still
running but busy ?

Have you rebooted the NAS to make sure MySQL really is dead before you
start SlimServer ?

If you have the database backup left since the earlier tests, you can
try to restore that and reboot the NAS and then wait until it has
created the database index after you have started SlimServer. It was
pretty fast to create the index in my own setup, but I would expect
that on a NAS and with a larger library it could at least take a number
of minutes. It will be a lot faster than that if the database is empty
at the first start up with the new Custom Scan version.
