What the modified custom-convert.conf file does is send WAV to STDOUT but an unmodified mplayer will also send text messages to STDOUT so you may get bursts of noise in the output stream - usually at start and finish.
The mplayer-stdout patch redirects all text messages from mplayer to STDERR and not STDOUT.
