I read somewhere that the GROUP tag in iTunes was originally intended to accomplish this. I guess you would tag tracks on an album that should be grouped together with some unique identifer in the GROUP tag and then iTunes would know to play them together. I'm not sure this actually works, but apparently that was the idea, and it seems like something like that could be done.
