You can do it with Dynamic Playlist plugin together with SQL Playlist
plugin.

With SQL Playlist plugin you can create a playlist that requests a
parameter from the user when it is played. If the parameter is of one
of the standard types (artist, album, year, genre, playlist) it can be
launched from the standard browse menus by holding play down and
selecting the "Dynamic Playlists" mixer. It can also be launched from
the web interface by hitting the PL icon beside the artist/album. 

There are a lot of different playlist templates provided with the SQL
Playlist plugin, but at the moment there is no one that requests
parameters from user and limit the result to minimum rating and songs
that haven't been played recently. If you know SQL you can create your
own playlist already now by creating a playlist and customizing the SQL
, if you don't the next version of "SQL Playlist" will have support for
requesting parameters in the "Advanced" and "Advanced with rating"
playlist templates which should do exactly what you requested.
