If it skips I would suspect that you have an incorrect filter in the Custom Skip plugin. The Dynamic Playlist and SQL Playlist plugin should only cause problems if you are playing a dynamic playlist when you have this problem but it sounds like you have it always ? Custom Browse is only suspected if you have this problem only in menus based on Custom Browse, but I'm guessing that's not the case ?
