It's ok, as I mentioned at the bottom of my post, it was caused by Spotify creating two accounts and logging me into the wrong one in my web browser (I suspect due to a cookie or session issue). I inadvertently upgraded the wrong account to Premium - even though I had logged in with that username/password, it actually picked up the Facebook account instead. I've resolved the issue by switching the Premium membership over to the username/password account.
