It's not possible to do with just Custom Skip, Custom Skip will look at
each skipping criteria individually and if it's matching it will skip
the track.
However, if you also use Multi Library plugin, you can define a virtual
library with a custom SQL that defines your combined criteria and then
you can in Custom Skip add a skipping criteria using the "In Library" or
"Not in Library" skipping methods. The specific criterias you mention is
not part of the templates provided with Multi Library, but you can
create your own library definitions using SQL and then you can more or
less do anything you want as long as the information is in the database.
If you want to try this, install Multi Library plugin and apply for the
free 30 day evaluation license through License Manager (unless you
already have a license) and after that verify that you get it to work
with a simple library definition. If you after that want to create
something based on a custom SQL, just post here and me or someone else
will try to help defining it.
Also, please keep in mind that Custom Skip only works good if most
tracks aren't skipped. If you use Custom Skip to skip during
smart/random playlists, it might be preferred to use SQL
Playlist/Dynamic Playlist plugins instead of Custom Skip if you want to
have a skipping criteria that's going to skip a lot of tracks.
