Re-entry after any time increases the volume. If done immediately, the increment is 5. A longer interval can cause an increase which is a multiple of 5. You see the volume slider move to the right shortly after re-entry (as well as hearing the volume increase, of course). No Bluetooth speakers, Airplay or other external audio component that I can think of. There are a couple of iPads though, at least one of which has playback enabled in iPeng. I haven't tested whether the same thing happens with either of them but can do so tonight if it might help with diagnosis...
