That iPhone behavior is normal, the iPad behavior is probably a
coincidence.

The volume iPeng uses for its player is server-controlled so whenever
iOeng starts the player, the volume is set to the volume the server
sends, that's consistent with the behavior of a squeezebox and allows
remote control of the volume.
iPeng remembers the volume the device was set to before you opened iPeng
and restores this level when you quit it.
Your iPhone was probably set to 100% volume before you started iPeng and
that's where it reverts to.
It's of course different if iPeng still plays, then the playback volume
still remains valid, it should change when playback stops (actually five
minutes later, when iPeng goes to sleep).

I'll have to check whether there is some kind of edge case where the
volume change is handled wrongly if the volume was set to 100% before
you enter iPeng.
