I have no idea why the exit pop-up stopped showing up. Maybe Amazon changed something in the FireTV recently and app no longer gets notified of a "Back" key press when the user tries to exit the app. I don't have a FireTV so I can't test it directly. I'll do some googling to see if I can find anything about this problem.
In the mean time, just disconnect before you exit. Resource usage wise it will basically be the same since Android doesn't fully remove an app from memory even when you exit it. As to the player list, LMS seems to like to keep disconnected players from the list for a bit of time but it should eventually remove it. I think it does this in case the player is just disconnected momentarily. For the 'command stream error', try increasing the Network Timeout in Settings > General.
