It only loads the covers on the currently showing list. Items on a list are loaded incrementally about 200 at a time, but cover art is loaded about 20 at a time.
For track lists of the local music library it will also make a second request to the server for each 200 item block, to retrieve extra info for each item (album gain, encoding format, duration, etc.) As to why Squeeze Ctrl has to make a 2nd request is beyond the scope of this post. For track lists of plugins and "My Apps" apps, no second request is made.
