I also have seen the LMS server log error "Decoder does not support file format, code 0" on occasions, sometimes several in a row. Once in a while my M1 CLiC renderer and sq2u get completely out of sync and I end up having to stop everything, powering down the renderer and restarting everything.
The only clue I've got as to why is that I think it has sometimes occurred when I have previously been using a different server and UPnP Control Point to play on the renderer, and that it's possible the systems have interfered with eachother (which would be no surprise). LMS 7.9 on VortexBox Midi box, Xubuntu 14.04, FLACs 16->24 bit, 44.1->192kbps. LMS plugin UPnP/DLNA Bridge to MF M1 CLiC (to 405/2 amp & ESLs) & Marantz CR603 UPnP renderers. Alternatively Minimserver & Upplay to same & to upmpdcli/mpd PC renderers. Squeezelite to Meridian USB Explorer DAC to PC speakers/headphones. Wireless Xubuntu 15.10 laptop firefox/upplay or Android 'phone with Squeeze-Commander/BubbleUPnP controls LMS/Minimserver.
