This has been discussed several times on this list. The final conclusion was 
that this is a bug of Mapscript that creates (or returns) wrong temp files 
names (sometimes takes 'older' already existing ones). Seems that it is still 
not fixed.  

In pmapper 2 (latest dev snapshot should fully work) I have modified all 
requests to saveWebImage() that causes this problem to a new defined custom 
function in common.php. You can use the same for pmapper version 1. After that 
modifications this error did not appear any more.
