I thought I dealt with that, but obviously not sufficiently!
Please modify page NewContact in this way:
change the lines for first name and surname to this:
|| '''First Name:'''||(:input text name1 "{$:name1}" size=60:) ||
|| '''Surname:'''||(:input text name2 "{$:name2}" size=60:) ||
{$:name1} and {$:name2} need to be in double quotes.
I've updated the recipe.
