Hi,

You might want to check podofo/test/FormTest. This tests shows how to create 
PDFs with forms, but reading existing forms and changing should works 
similar.

PoDoFo's form field support is not yet complete, but modifying text fields and 
check boxes should work very well. For comboboxes some extensions might be 
needed.
