I don't normally reply to myself but I have finally found the solution to
the problems in the last paragraph below.

For Hotmail accounts that end in @msn.com, go inside OE and set the HTTP
Server URL to:

http://oe.msn.msnmail.hotmail.com/cgi-bin/hmdata

Note that accounts which end in @hotmail.com are set to:

http://services.msn.com/svcs/hotmail/httpmail.asp

If you want your Hotmail account that ends in @msn.com to be automatically
setup in OE then visit this site:

http://supportservices.msn.com/us/oeconfig/

Fill out the short form, click OK and you're done! Almost as good as
POP3/SMTP service.
