I'll leave most of the rest to others, I don't use dovecot, local
mail-handling is done by Panda-IMAP, and I must really knuckle down and
build the SSL version of it one of these days...
I use postscreen, fail2ban, and the various SPAMhaus tools to keep the
incoming dross to a minimum.
I report a lot of the SPAM that does get through to SPAMcop.
I use SMTP2go to handle my own domain's outgoing email, and Mailroute
for stuff from my ACM address.
