I think there is a typo in the file:

/^ip[12]?[0-9]{1,2}(-[12]?[0-9]{1,2}){3}\.adsl2?\.static\.versatel\.nl$/       
PREPEND X-GenericStaticHELO: (versatel.ml)
should read /ml/nl/
/^ip[12]?[0-9]{1,2}(-[12]?[0-9]{1,2}){3}\.adsl2?\.static\.versatel\.nl$/       
PREPEND X-GenericStaticHELO: (versatel.nl)
