I find this strange:
I don't see 216-188-250-60.dyn.grandenetworks.net[216.188.250.60] in
mynetworks, while sends helo=192.168.129.6.

It looks like the client was moved from intranet to Internet or should be
connected to a local vpn.

I have a few virtual domains in my postfix config, and they've suddenly
started refusing to send (they receive fine) with an access reject. I.E.
