Not sure what you mean by sometimes.] I have found that it has missed icons once or twice for me, and from examining the logs, there does appear to be a timing issue. There are actually two data structure of icon info maintained: one in pprotray and a separate one in powerpro for performance (eg when drawing a bar to have immediate access to tool tip and icon). It appears that if a new icon gets created while PowerPro is asking pprotray for initial copy of database, then there could be problems but not sure of the exact cause or fix for now.
