Most of Neil's registry tips are very well written but that one is extremely
confusing. However googling for other tips describing the same behavior your
description is in sync with what I understood it to mean and with how I have
my .ico registry entries setup. I've tried a TweakUI Repair Icons and it
just doesn't seem to take.
Here's the top level .ico entry:

I don't know what if anything the PersistentHandler key does or what it
comes from - it occurs in many but not all of the HKEY_CLASSES_ROOT\.xxx
extension keys - perhaps a red herring?

I had hopes that this would be the missing link since when reading your full
description, I was pretty sure that's how I already had it set up. As you
can see from the above icofile key (for me "IrfanView.ico") has no such
CLSID number.
Can you spot anything I'm missing in the above setup or any other ideas?
