I recently purchased QuickFolders from ByteGems (www.bytegems.com), 
which does everything you've mentioned rather nicely.

I actually didn't realise until after purchasing, however, that PP does 
all this... last time I looked it wasn't as easy to do in PP (?), or I 
didn't see the Help section when I first looked, or something like that.

Now that I've got QuickFolders, though, I'm using it & really liking 
it.  It does "embed" its menu into the Open/Save dialogs as you're 
wanting, plus it has a few other handy features that I like.  Big 
footprint, however -- usually eats ~1.5mb of memory (and ~6.5mb of virtual).
