4 & 5, the center pins are the modem.  Although it is an RJ-45 
connector an RJ-11 cable will plug in to it (it was designed that 
way) and the center pins on a phone cable are line 1.

About two years ago I found a replacement Ethernet / modem for a PB 
G3 which is the same for the 3400.  They are out there.
