Damaged ribbon cable?
Can you used a continuity tester end to end on every pin placement?
Bad interface? SCSI? IDE/ATA ?
Do you have or can you borrow an external drive to connect to an external bus and see if there is the same problem?
I forget can the drive be tried on another machine?
You don't really say if you tried a reformat.
But if I recall correctly you are trying to preserve data on that disk? You don't want to initialize? Right?
