STP would like to know the following Dosimetry Benchmarking Questions:
1. Do you require a detailed lifetime exposure records, NRC Form 4, when personnel process in? 2. Do you require a written record from each licensee, NRC Form 5, for the current year when personnel process in? 3. Do you utilize PADS when the new individual does not have a dose estimate? 4. Approximately how many man-hours are spent on processing one person in; including time it takes person processing in to provide documentation? 5. How many persons, techs and professional staff are involved in your dosimetry program during outages and normal operations?
