3.  Do your Maintenance Planner's develop job dose estimates?     Yes____   No_____
4.  Does your facility have daily dose goals?    Yes____  No____      or  Weekly goals?   Yes____  No____
5.  Does your facility track dose to the tenth of a mrem?   Yes____   No____
6.  Do you use wrench time for your estimates?   Yes____   No____    or  Total RCA time?   Yes____   No____
7.  Do you use a % of the hours provided by Maintenance Planning to develop your estimate?   Yes____   No____  (ie. 30%)
