Palo Verde experienced a similar issue last year during our fall refueling 
outage while lifting the reactor head for placement on the head stand.  The 
lift was done with improper alignment which resulted in the head rubbing 
against the guide pins (OE 35094, 34668).  This required inspections and repair 
of the guide pins and Reactor head.

As far as radiological controls, we included misting installed equipment to 
keep high contamination from going airborne, maintenance used an electronic 
scraper to minimize the potential for airborne activity, we set up a tented 
area with HEPA filtration on the exhaust to perform repairs and we performed 
decontamination before surface destruction activities were permitted. Initial 
survey of the guide pins were 300,000-1,500,000 dpm/100cm2. Post 
decontamination surveys were less than or equal to 1000 dpm/100cm2.  Our RWP 
contained a hold point to perform a TEDE ALARA evaluation before de-burring 
activities.
