From Callaway If the incoming radworker presents a properly completed and signed NRC Form 4, then we will issue a dosimeter. If the radworker does not have a completed Form 4, then we will provide them a PADS Form 4 for their review and signature. We don't normally issue dosimeters after hours, except by special arrangement and during outages. Only the dosimtery clerk is trained for PADS.
