Hope all is well... I have answered your question based on the way we are 
utilizing the inserts at Fermi 2:

1.            Are the areas you are using the escort required insert always 
associated with high dose rates and infrequent access?  We only use the Escort 
Required for areas of high dose, infrequent access, and for areas that we know 
we have some type of transient in progress, or imminent.


2.            Do you use the "RP Brief Required for Entry" on not routinely 
surveyed RA's as well as HRA's and LHRA's?  We utilize RP Brief Required for 
HRA/LHRA as well as for RA posted areas that are greater than 25 mrem/hr and 
for contaminated areas that are greater than 10K dpm/100cm2



3.            When a room is posted with the escort required insert, do you 
require an escort with every entry even if you have an updated survey and know 
conditions will not change?  We do not require an escort for every entry if we 
have updated surveys, but we do initiate a Job Coverage Record to document that 
we used survey "xxxx" to provide the workers their briefing.

Comanche Peak would like to benchmark how other stations are utilizing the "RP 
Escort Required for Entry" and "RP Brief Required for Entry" posting inserts. 
In addition to HRA's our plant has been utilizing the inserts in areas that are 
not necessarily high dose, but infrequently accessed, therefore, not routinely 
surveyed.
