I have a question on how other healthcare providers are handling the
Minimum Necessary issues when it comes to third party providers, like
radiologist, pathologists, anesthesiologist, ...  These doctors are
involved in patient care, and need information such as demographics,
insurance, ... in order to bill for their services, yet they may never see
the patient.

Most HIS systems do not handle patient security for this type of specalist
very well in that there is not a direct doctor - patient relationship.
Therefore, you can not give the doctor access to "his" patients, since the
doc is not listed as a doctor for the patient.

The question, how are other healthcare providers allowing access to the
required information, without allowing access to the entire patient
database? Or can those specalists have access to the complete database,
even if they have only been involved in the treatment of some?
