Doctor pro - a photoshop plugin from colorvision should do the trick for
you. If you want to go more advanced Profile Editor from GretagMacbeth is a
more robust solution.
I must say though, that I rarely find profile adjustment nessesary, on
properly set printers, provided you didn't make any measurement errors.

I'd try first off to do multiple measurements of the chart and
compare/average. Also remember to make a custom softproof for really
accurate viewing, preperably with inkblack/paper white ON.

If this doesn't cut it, and renewed monitor profiling doesn't do it either,
then it's on to editing...
