That was a long time ago...
 
Most recently (about a year ago)  I wound up starting up Adobe and then 
printing that way.  Pretty ugly because Adobe wouldn't always close itself 
<sigh>
 
Check out visagesoft at http://www.visagesoft.com/index.php I downloaded the 
trial ware and had PDF's viewed and being printed in about five minutes... 
Looks painless, albeit a little pricey.
